Requirements
Referrals from outside the MTF should have copy of or CD of radiology studies, labs, diagnosis, study findings, brief clinical description or reason for consult.
Clinic Information
- Beneficiary Category: All
- Beneficiary Ages: Adult 18 years and older. Pediatric referrals require direct physician-to-physician contact
- Urgent Referrals: Requires direct physician-to-physician contact
- Schedule Release: 6 months in advance
- Patient Instructions: Check in at least 15 minutes prior to appointment
- Additional Information: New consult must be entered if patient has not been seen in the clinic within 3 years.
