Operational Forces Medical Liaison (OFML) & Medical Evacuation (MEDEVAC)
NMRTC San Diego Operational Forces Medical Liaison Office Mission:
To provide 24 hours per day, seven days per week point of contact between NMRTC San Diego and the operational forces.
NMRTC San Diego MEDEVAC Office’s mission:
To coordinate all administrative and logistical procedures incidental to the aeromedical evacuation (fixed-wing MEDEVAC) of patients to and from San Diego.
*Rotary-wing MEDEVACs (helicopter) arriving to NMRTC San Diego are coordinated by the Officer-of-the-Day at (619)572-6323.
Forms description:
OFML will assist in submitting referrals/consults/labs IF UNDERWAY
- Please communicate in your request that you are underway.
- Fill out SF 513 for consults, SF 519 for radiology referrals, ADMIN-049B for laboratories. We recommend sending the documents encrypted.
- Include patients full name, DOD ID, DOB, PT phone number, Provider NPI and signature.
-Provide reason for request, provisional diagnosis, and specialty clinic where patient will be referred to. Each clinic has their own requirements and scheduling.
- Once form is completed please email form to the OFML office group email at usn.san-diego.navmedcensanca.list.ofml-nmrtc-sd@mail.mil Once received and processed OFML will confirm submission via e-mail to the referring provider.
