Appointments

All services with the exception of audiograms, are provided on an appointment basis. Please call the listed phone number to get information regarding scheduling appointments. Employees should bring a Dispensary Permit (OPNAV 5100/9) or "Supervisor's Medical Surveillance and Certification Exam Referral" (SECNAV 5100/1) . The applicable portion of the form should be completed by your supervisor when coming to the clinic. All OHUs are open Monday through Friday, 0700-1530.
